Obesity as a Chronic Medical Condition
Obesity is now widely recognized by leading medical organizations as a chronic, complex disease not a lifestyle failure or a matter of insufficient willpower. It involves dysregulation of the hormonal, neurological, and metabolic systems that govern hunger, fullness, energy storage, and metabolism. Left untreated, obesity significantly increases the risk of type 2 diabetes, cardiovascular disease, obstructive sleep apnea, fatty liver disease, certain cancers, and joint disease, among many other conditions. Effective, evidence-based medical treatment including FDA-approved medications used alongside dietary changes and physical activity is now available and represents a meaningful advance in how healthcare approaches this disease. Two prescription medications currently available for chronic weight management are Zepbound (tirzepatide) and Foundayo (orforglipron).

Weight Loss Treatment
Zepbound® (Tirzepatide Injection) — A Dual-Action Weekly Injectable
Zepbound (tirzepatide) is a once-weekly injectable prescription medicine FDA-approved for chronic weight management in adults with obesity (BMI ≥ 30) or overweight (BMI ≥ 27) with at least one weight-related health condition. It is also approved for the treatment of moderate-to-severe obstructive sleep apnea in adults with obesity the first medication ever approved for this indication. Tirzepatide works as a dual agonist, activating both GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic polypeptide) receptors. This dual mechanism reduces appetite, slows gastric emptying, and influences how the brain processes hunger and food reward. In clinical trials, patients on the highest dose of tirzepatide lost an average of more than 20% of their body weight over 72 weeks. Zepbound is intended to be used alongside a reduced-calorie diet and increased physical activity, and it is designed for long-term use as part of an ongoing weight management strategy. Patients should discuss all health conditions and current medications with their provider before starting, and should be aware of important safety information including warnings about thyroid C-cell tumors and contraindications in patients with a personal or family history of medullary thyroid carcinoma or MEN 2.
Foundayo™ (Orforglipron Tablets) — A Once-Daily Oral Option
Foundayo (orforglipron) is a prescription oral tablet taken once daily for chronic weight management in adults with obesity or overweight with weight-related health conditions. It represents a significant development in the GLP-1 drug class: unlike semaglutide and tirzepatide, which require weekly injections, orforglipron is a small-molecule GLP-1 receptor agonist that can be taken in pill form, without the refrigeration requirements and injection administration of earlier GLP-1 medications. This makes it a potentially more accessible option for patients who prefer an oral route or have concerns about injectable therapies. Foundayo should be used together with a reduced-calorie diet and increased physical activity, and it should not be combined with other GLP-1 receptor agonist medicines. As with injectable GLP-1 medications, it carries a warning regarding thyroid C-cell tumors and is contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). It is not known whether Foundayo is safe and effective for use in children. Patients should consult their healthcare provider for full prescribing information and to determine whether Foundayo is appropriate for their individual health needs.
